There is a School Nurse at West Oak Lane daily when school is in session.

For your convenience, parents may now download many of the required health forms:
Asthma Action Plan – required for any student who carries medication for Asthma in school – must be signed by a physician and renewed every year- please keep extra medicine in Nurses’ office
Dental Form – required for any New student, 3rd graders and 7th graders – all are due upon the start of school – If they are not received by December 15th, your child will not be able to re-enroll for the following school year until paperwork is received by the Nurse.
Food Allergy Plan – required for those students with Food Allergies – must be signed by a physician and renewed every year – please be sure to send in Benadryl and/or Epi pen with forms.
Immunizations – must be up to day upon entrance to school – during the 6th grade Physical, students must receive TdaP and Meningococcal Vaccinations. Your child will not be able to re-enroll for the following school year until paperwork is received by the Nurse.
